RESUMO
The article describes experiments in animals with laser revascularization of the myocardium and first operations of laser transmyocardial revascularization of the myocardium in man without a combination with operations on the coronary vessels. Operations were performed on 15 IHD patients with the III-IV functional class of cardiac insufficiency. The operations were favourable in all the patients. Immediate and long-term results suggest that this operation can be an alternative to direct interventions on coronary arteries and seems to be the only possibility in patients for whom operations on the heart vessels are not indicated or contraindicated.

The authors have modified the well-known methods of anastomosing between hollow organs of the gastrointestinal tract using radiation and intestinal forceps with narrow branches. The amount of inflammatory and purulent complications decreased.

The authors developed a techniques of anastomosis formation after gastrectomy, proximal resection of the stomach, and resection of the descending colon and rectum in which the anastomosis begin functioning a few days after the operation. They called them autorecanalizing anastomoses. From the moment of the operation and to autorecanalization the lumina of the anastomosed organs are drained by original methods. The purpose of the developed techniques for the formation of autorecanalizing anastomoses and the methods for decompression and draining of the operated on organs is to reduce the risk of suture incompetence and the hazardous effect of this complication on the patient's life.

The authors modified the method for formation of aseptic anastomosis, using radiation of the CO2-laser and neodymium-doped YAG laser. The techniques for placing the enteroenteric anastomosis, gastroentero- and cholecystojejunoanastomosis are presented. Reduction in incidence of inflammatory and purulent postoperative complications has been achieved.

Use of a carbon dioxide laser to vaporize the local lesions caused by cutaneous leishmaniasis is reported. A total of 108 patients have been treated in this way and followed up. The treatment reduces the management time of patients at least 1.5 times and is followed by satisfactory aesthetic outcomes. No recurrences have been observed among the 82 patients who have been followed up for 7 years.

A novel technique of 'autorecanalizing' anastomosis has been developed experimentally to reduce the risk of anastomotic failure when resection of the left colon or rectum is performed in the face of suboptimal bowel preparation. The method has been used in nine patients who presented with large bowel obstruction. One patient developed a low output faecal fistula that closed spontaneously within 19 days of operation. No leakage was detected in the remaining eight patients. The delay between operation and recanalization ranged from 8-13 days and the caecostomy used for intubation closed spontaneously within 24 days of surgery.

During a ten-year period 642 abdominal hollow organ resections have been performed by means of a new, original technique--that is, CO2 laser irradiation combined with specially designed instrumentation. This resulted in a remarkable decrease in postoperative morbidity and mortality. The article describes the laser procedure.

An original method of delayed anastomosis of the colon is proposed. The use of the proposed kind of anastomosis might be an alternative to the Hartman operation for carcinoma of the left half of the colon complicated by intestinal obstruction.
